App Description
This application helps you monitor your health and stay in communication with your healthcare team. This is a subscription service in which you must be pre-enrolled with a designated username and password by a healthcare facility. In the application you may be asked questions and given information about your health. With the new version you will be able to send images with your healthcare information.
