Download athenaWell.
Sign in using your patient portal credentials.
Select the conditions you are monitoring and explicitly grant permission for which data types to send to your health record, where it can be reviewed by your doctor. athenaWell supports sharing the following data types from Health: blood glucose, blood pressure, heart rate, steps, and weight.
Use of athenaWell is currently available only to patients of Hudson Headwaters Health Network of New York. For those interested in learning more about athenaWell, download the app to experience a walkthrough and additional information.
