WHAT IS AMAZE™?

AMAZE™ is an app for current asthma or heart failure patients at participating healthcare providers. This app lets you access your protected health information and gives your doctor better insight into the ups and downs of living with either asthma or heart failure. Once you log into the app, you’ll get a custom experience for your health condition.

This app gives you the tools and education to track, manage, and make shared decisions with your doctor. The app works best with continued, daily use so that your doctor can make the best health decisions for you.

HOW DO I USE IT?

Daily log:
Each day, you can complete a log which asks about your symptoms, medication use, ER and urgent care visits, vitals, and more! Your daily log answers are automatically shared with your care team so they know how you are doing between doctor appointments.It takes just a few minutes each day to share invaluable information about your health condition with your care team.

Trends:
AMAZE™ will help you stay on trackwith your daily log goal. View your progress by day or week which helps you see what you logged and what you missed. The app can also remind you to submit a log before the day’s over. As you submit a log each day, you’ll build up your streak count. How long can you keep up that streak?

Education:
AMAZE™ provides access to trusted, in-depth articles which are easy to read and understand. Knowing more about your health condition is an important part of your overall healthcare.

Medications and Reminders:
Stay up to date with the medication your doctor prescribes to you. Never forget to take your medication with the ability to set up as many medication reminders as you need. You also have access to in-depth information about each medicine, your preferred pharmacy, the prescribing doctor, and the ability to report adverse events.

Appointments:
You’ll always know when your next appointment is with a list of your appointments -right from the app.

Quality of Life surveys:
Periodically, AMAZE™ will ask you to complete a short survey about your health condition. Your answers to the survey are shared with your care team to help them understand how your health condition changes over time.

Message your care team:
Have a question about a symptom or how to take your medication? You can always send a secure message to your care team at any time of the day. Messages are not monitored in real time but are regularly reviewed.

Air quality:
For those who have asthma, the app gives you local, hourly air quality conditions plus common allergens such as trees, grass, and ragweed. Search air quality conditions for anywhere in the world -great for planning ahead!

Your data is secure on our HIPAA compliant, HITRUST certified platform.
